Au cours de ces dernières années de nouvelles technologies (analyse en cellules uniques, cytométrie multidimensionnelle, analyse multiplexe in situ…) ont permis de définir de nouvelles sous populations de LT-CD8 et de mieux comprendre les cellules ciblées par les traitements d’immunothérapie par anti-PD-1 ou anti-CTL14 ou dans le cadre de vaccination anti-tumorale. Suite à de nombreux travaux, deux principales populations ont émergé, les LT-CD8 résidents mémoires (TRM)(CD103, CD49a, CD69) et les LT-CD8 épuisés progéniteurs (TCF1) comme jouant un rôle prédominant dans le mécanisme d’action des anti-PD-1. Ces 2 types cellulaires peuvent s’expandre après traitement par anti-PD-1 et des études convergent pour leur conférer un rôle de biomarqueur prédictif de réponse à l’immunothérapie. Les LT-CD8 résidents mémoires sont nécessaire pour l’efficacité thérapeutique de vaccins anti-tumoraux. L’induction de ces TRM nécessite une voie mucosale d’immunisation et l’identification des cellules dendritiques (CD103, Clec9a, CD1c, CD63) à l’origine de leur activation stimule différentes approches thérapeutiques ciblant ces cellules présentatrices d’antigènes. Des progrès récents dans l’analyse du rôle du ganglion dans l’efficacité des traitements par anti-PD1/PD-L1 via l’activation d’une population de LT-CD8 épuisée progénitrice pourraient aussi déboucher sur le développement de traitements néoadjuvants pour optimiser l’efficacité de l’immunothérapie avant l’exérèse chirurgicale des ganglions associés à la tumeur.
